Fill out the form and the downloadable PDF form at the bottom of the page. Upload the following documents, each in its own dedicated box, and forward the request.
1. Completed and signed form (MANDATORY)
2. Copy of the payment receipt (MANDATORY)
3. Copy of an identity document (MANDATORY)
4. Passport photo (OPTIONAL, you can send/deliver it later to the general secretariat of your reference office)
We also remind you that the original documents must be physically delivered to the Secretariat at the beginning of the Academic Year.
